W SZKOLE DOKTORSKIEJ W DYSCYPLINIE NAUKI MEDYCZNE W ROKU AKADEMICKIM 2022/2023
która będzie miała miejsce w dniach

Warunki i tryb rekrutacji do Szkoły Doktorskiej Śląskiego Uniwersytetu Medycznego
w Katowicach w roku akademickim 2022/2023 określa Uchwała nr 7/2022 Senatu
z dnia 23.02.2022 r.

Postępowanie rekrutacyjne składa się z trzech etapów:

  1. Ocena weryfikacji kompletności złożonych przez Kandydata w elektronicznym systemie wymaganych dokumentów oraz dokonanie oceny Kandydata zgodnie z Arkuszem oceny kandydata (do 31.08.2022 r.)
  2. Rozmowa kwalifikacyjna z Kandydatem ( 05-09.09.2022 r.)
  3. Wpis na listę przyjętych doktorantów do Szkoły Doktorskiej albo wydanie decyzji administracyjnej o odmowie przyjęcia do Szkoły Doktorskiej (do 30.09.2022 r.)

Wymagane dokumenty niezbędne do dołączenia do elektronicznej rekrutacji:

  1. Podanie o przyjęcie do Szkoły Doktorskiej (pobierz tutaj);
  2. Curriculum Vitae (CV) wraz z zawartą zgodą na przetwarzanie danych osobowych na potrzeby postępowania rekrutacyjnego;
  3. Odpis dyplomu lub zaświadczenie o złożeniu dokumentów niezbędnych do wydania dyplomu ukończenia studiów drugiego stopnia, jednolitych magisterskich albo odpisem dyplomu studiów pierwszego stopnia lub zaświadczeniem o ukończeniu trzeciego roku jednolitych studiów magisterskich wraz z dokumentacją potwierdzającą najwyższą jakość osiągnięć naukowych a w przypadku cudzoziemców odpis dyplomu potwierdzającego ukończenie uczelni wyższej za granicą, która daje prawo do ubiegania się o nadanie stopnia doktora w danym państwie,
  4. Zaświadczenie (lub suplement) o średniej arytmetycznej ocen z całego toku studiów;
  5. Kolorowa fotografia o wymiarze 40×55 mm;
  6. Poświadczone przez Kierownika właściwego Dziekanatu oświadczenie Kierownika Katedry (lub innej samodzielnej jednostki nauczającej), w której doktorant ma odbywać kształcenie, że w kierowanej przez niego jednostce możliwe jest odbywanie obowiązujących zajęć dydaktycznych (pobierz tutaj);
  7. Oświadczenie Kierownika Katedry, w której doktorant ma odbywać kształcenie, że istnieje możliwość przeprowadzenia badań naukowych niezbędnych do realizacji planowanej rozprawy doktorskiej (pobierz tutaj);
  8. Dokument potwierdzający znajomość języka angielskiego w formie certyfikatu lub dyplomu lub oświadczenie o znajomości języka angielskiego w stopniu umożliwiającym kształcenie w Szkole Doktorskiej, w której wiodącym językiem nauczania jest język angielski (pobierz tutaj);
  9. Wykaz osiągnięć kandydata (pobierz tutaj) wraz z kserokopiami dokumentów potwierdzającymi te osiągnięcia. Doniesienia zjazdowe muszą być udokumentowane certyfikatem wygłoszenia pracy i/lub streszczeniem w materiałach zjazdowych, a publikacje pełnym tekstem pracy. Każdorazowo należy dołączyć kserokopię strony tytułowej czasopisma, książki, referatów zjazdowych lub streszczeń;
  10. Dokument wykazujący punktację uzyskaną z  posiadanych publikacji potwierdzony przez Bibliotekę.
  11. Zaświadczenie lekarskie zawierające orzeczenie o braku przeciwwskazań do podjęcia kształcenia w Szkole Doktorskiej, wydane przez lekarza spełniającego wymagania kwalifikacyjne określone odrębnymi przepisami. Skierowanie dotyczące badań lekarskich można odebrać osobiście w Kolegium Szkoły Doktorskiej;
  12. Ogólna koncepcja planu badawczego (pobierz tutaj);
  13. Karta informacyjna kandydata (do uzupełnienia, podpisania i późniejszego wydrukowania z systemu elektronicznej rejestracji)

 

Kandydaci, którzy uzyskają wymaganą liczbę punktów z Arkusza oceny kandydat (min. 20 pkt) zobowiązani są do dostarczenia osobiście do Kolegium Szkoły Doktorskiej SUM w terminie wskazanym przez Komisję wymaganych oryginałów ww. dokumentów.

ELECTRONIC RECRUITMENT FOR THE FIRST YEAR OF EDUCATION
IN THE DOCTORAL SCHOOL IN THE
MEDICAL DISCIPLINE
IN THE ACADEMIC YEAR 2022/2023

The conditions and procedure of recruitment to the Doctoral School of the Medical University of Silesia in Katowice in the academic year 2022/2022 are set out in Resolution No. 7/2022 of the Senate of February 23 2022.

The recruitment procedure consists of three stages:

  1. Assessment of the completeness verification of the required documents submitted by the Candidate in the electronic system and assessment of the Candidate in accordance with the Candidate Assessment Sheet (until August 31, 2022)
  2. Interview with the Candidate ( 05-09/09/2022)
  3. Entry on the list of doctoral students admitted to the Doctoral School or issuing an administrative decision refusing admission to the Doctoral School (until 30/09/2022)

Required documents necessary to join electronic recruitment:

  1. Application for admission to the Doctoral School (download here);
  2. Curriculum Vitae (CV) with the consent to the processing of personal data for the purposes of the recruitment procedure;
  3. A copy of the diploma or a certificate of submission of documents necessary for the issuing of a second-cycle diploma, uniform master’s degree or a copy of the first-cycle diploma or a certificate of completion of the third year of uniform master’s studies, together with documentation confirming the highest quality of scientific achievements, and in the case of foreigners, a copy of the diploma confirming completion a university abroad which gives the right to apply for a doctoral degree in a given country,
  4. Certificate (or supplement) with the arithmetic mean of the marks for the entire course of study;
  5. Color photograph, dimensions: 40×55 mm;
  6. A statement from the Head of the Department (or other independent teaching unit) in which the doctoral student is to be educated, certified by the Head of the relevant Dean’s Office, stating that the unit under his supervision is possible to attend obligatory didactic classes (download here);
  7. Statement by the Head of the Department in which the doctoral student is to be educated that it is possible to conduct scientific research necessary for the implementation of the planned doctoral dissertation (download here);
  8. A document confirming the knowledge of the English language in the form of a certificate or diploma or a declaration of the knowledge of the English language sufficient to enable education at the Doctoral School in which the leading language of instruction is English (download here);
  9. List of the candidate’s achievements (download here) along with photocopies of documents confirming these achievements. Reports from the congress must be documented with a certificate of the presentation of the paper and / or a summary in the materials of the congress, and publications with the full text of the paper. Each time a photocopy of the title page of the journal, book, conference papers or abstracts should be attached;
  10. Document showing the scores obtained from the publications held and confirmed by the Library.
  11. A medical certificate containing a statement that there are no contraindications for undertaking education at the Doctoral School, issued by a doctor who meets the qualification requirements specified in separate regulations. A referral for medical examinations can be collected in person at the College of the Doctoral School;

  13. Candidate’s information sheet (to be completed, signed and later printed from the electronic registration system)

Candidates who obtain the required number of points from the Assessment Sheet, the candidate (at least 20 points) are required to deliver the required originals of the above-mentioned documents personally to the College of the Doctoral School of SUM within the time limit indicated by the Commission documents.
